of the following forms must be completed and returned to the Board. A check
for the rower's fees may be included with the completed forms, or fees may
be paid on-line. Registration is complete when all forms and dues have been
collected. Rowers with incomplete registrations will not be permitted to
participate in Crew activities.

1. Application to Row and Join the FHS Crew Club |
|
This form must be completed. Parents/Guardians must give written permission for their student to row. As members of the crew team both students and their families also pledge to volunteer for fund raising, regatta jobs, and other Crew Club activities.

2. Virginia HS League Athletic Participation & Physical Exam Form (4 pages) |
|
This form is required for athletic eligibility. Every rower must have an Athletic Participation form on file for the current academic school year. If you already have a current form on file, complete the form below.

3. Emergency Care Information |
|
This
form is kept on file by the coaches at the boathouse and accompanies the
rower should medical attention be needed. All line items must be completed.
Attach additional sheets of health or emergency information, if needed.

|
4. US Rowing Release & Waiver Form |
|
Signed
form is required for all rowers who participate in regattas sponsored by
the NCASRA and US Rowing.
